RSVP Required 

The Caddo Area Council Physicals & Swim test is on April 2nd, 2026 from 5-8 PM. Please have your health forms printed and the attached documents pre-filled on the event day. 

Scouts are prepared!  Be sure to sign up in advance and arrive at least 10 minutes before your scheduled appointment.  When you arrive be sure all appropriate parts are filled out completely.  If you are late you will forfeit your appointment time and be asked to wait until we have an opening.

Parts A and B are required for all Scouts and adults participating in any Scouting events. This is the one the adult or the Scout’s parents fill out. Parts A and B cover contact information, insurance coverage, health history, hold harmless release, photo release, and adult or parent signature.

Do I need Part C?....Maybe.

Part C is the physical and required a physicians signature. Part C is required for Scouts and adults if any of the following apply:

• The event will last more than 72 hours
• The event is a high-adventure base event
• The event is strenuous and demanding

Part D only applies to Scouts and adults who are going to a high adventure base.

Scouts and adults who will be participating in a backcountry activity need to pay particular attention to the height and weight regulations on a form.
