MEDICAL INFO
It is very important that you have with you any medication you need on a regular basis. The Annual Medical Health Form (parts A & B) is required  and must include a parent/guardian signature for youth. 

The Circle Ten Screening and Statement (At-Risk Statement and Pre-Event Screening Statement) must be submitted at check-in by each person attending an OA event. The Commitment to Transport must be submitted at check-in for each youth. In case you are not caught up to our Covid-19 safety procedures for in camp events, a link to those can be found here.
