Youth AND adults will need to bring medical forms A and B, as well as a front and back copy of your insurance card. These will be returned to you at the end of the day. We will have blank forms day-of, but to expedite check-in, the following link will take you directly to the form. Please complete and print this for camp. Annual Health and Medical Record.
