Medical Form Review

Troops can turn in their Scout or Leader's Annual Health and Medical Records (parts A, B & C) prior to arriving at Camp Cedars. One leader can bring in all the youth and adult forms (or as much as is ready for turn-in). Forms must be the BSA Annual Health and Medical Record, no substitutions.

Collecting these forms ahead of time will help expedite the check in process for you at camp.

This is also a great opportunity to catch any mistakes on the forms prior to your arrival at Camp Cedars.  We will look for the completeness of the forms (parent signatures, Tetanus immunization within 10 years, physical examination within 1 year with medical professional signature, etc) along with scanned copy of insurance card.

Please remember all forms need to be in-hand and cannot be saved on a flash drive or electronic device.
