Notice to all candidates: All Ordeal candidates that have been elected during this election period should have received a written notice confirming their candidacy and an invitation to attend the upcoming Ordeal from the chapter.
Notice all Order of the Arrow members: An active Order of the Arrow member is currently registered with BSA and has paid 2022 Lodge dues. If your dues are not current, you can pay them online: https://scoutingevent.com/082-52773.
Notice to Elangomats: As an Elangomat, you agree to volunteer to lead candidates during the entire Ordeal Weekend. If you are interested in being an Elangomat for Ordeal Candidates, please contact us at aquiachapter@wipit470.org.

BSA Health & Medical Record Form required for all lodge/chapter events. All Order of the Arrow members/candidates who attend the Ordeal MUST complete the medical form. (It is the first one on this page; it is used for activities of less than 72 hours and does not require a doctor’s signature.)
All medications need to be in the original containers with the Scouts' name, dosage requirements (containers will be returned), and with the correct amount of Medication for the weekend. This includes over the counter drugs with a signed note from Parent/Guardian with dosage and instructions on time to be taken. Place all containers in a zip-locked bag labeled with the Scouts' name. All medications shall be turned in during check-in.
BRING THE COMPLETED HEALTH & MEDICAL RECORD FORM WITH A PHOTOCOPY OF BOTH SIDES OF YOUR INSURANCE CARD TO THE ORDEAL, IF THE FAMILY DOES NOT HAVE MEDICAL INSURANCE STATE "NONE".
Cancellation Policy: Refunds for events with fees of $25.00 and over may be granted (less 15% handling fee) for the following reasons: sickness (with physician's statement), relocation, or death in the family. If the event fees are under $25.00 per person, there will be no refunds and the fees cannot be credited toward another event. Refund requests must be submitted in writing to the Chapter Adviser and acknowledged.
